Some eye doctors use a form such as this one when they perform eye examinations. The examination form can be viewed either as a Word 2000 Document or as a Graphic Image:

After clicking on either one, if you would like to print it out, you first will need to set your margins, either in the word document window or in the browser window (on a PC), as follows:
Click on File > Page setup.
Click on the Margins tab (if there is one).
Set the margins as follows:
- Top = 0.50"
- Bottom = 0.50"
- Left = 0.25"
- Right = 0.25"
Print the exam form document.
After the form is printed, if it is not centered, or if part of one side of the form is missing, try adjusting the left and right margins accordingly. For instance, if about 0.1" of the left side is missing, then add 0.1" to the left margin and subtract 0.1" from the right margin. |
